Free Film and Video Production Forms
It takes organization to accomplish almost anything in the production environment, whether casting actors, booking a crew, securing a location or budgeting for post-production.
Luckily, you don’t have to re-invent the wheel when it comes to finding appropriate forms to help keep things moving smoothly along. Plenty of folks have been there before and are willing to share the forms and tools that helped them along the way.
Paul Zadie’s web site offers a page full of screenplay templates, storyboard sheets and production forms. Most of these are in Microsoft Word or Excel format. If you haven’t drunk the Microsoft kool-aid, try opening them in Google Docs. Zadie also comments (and I second the advise for all the resources listed in this post), “Please consult an attorney before using any of the release forms, agreements or contracts. I’m not responsible for anything you do.”
Dependent Films has a nice collection of forms and templates on their Forms and Utilities for Filmmakers page, many in Adobe Acrobat format. They also list some interesting downloadable goodies such as SMPTE Color Bars for your computer screen, a time-code calculator and a storyboard program.
The University of North Carolina School of Filmmaking web site lists dozens of film production related forms including checklists for cinematography, vehicles, casting, safety and script breakdown.
